Recently an individual has attempted to add fake or random student names to various accounts through Final Forms. The individual is not hacking into the Final Forms system but is accessing it much like a parent does when they are enrolling their first child in the district for the first time, which currently doesn’t require any authentication or authorization. The fake names were then linked to current students/parents through email addresses as if a parent was completing the paperwork to enroll a student. We are in the process of addressing the individual involved and the process for which the individual and all parents/guardians are able to access Final Forms.
If you receive an email asking you to complete the enrollment process of a random student who is not your child, please disregard it. Please know that we have found no evidence that any accounts have been accessed or compromised. However, if you have any concerns, please feel free to change the password to your Final Forms account. We apologize for any concern or inconvenience this has caused and thank you for your understanding in this matter.

Madison School Community,
Please be aware of the following situation that occurred in an area district this week. In a meeting with a student's parent, it was brought to that district's attention that her daughter was approached, at night, via Snapchat by individuals who represented themselves as a younger (age 17) female and a slightly older (20ish) male. They were encouraging her to move in with them where they would take care of her, feed her, take her where she needed to go, etc. They did not provide much other information, but the student indicated that she thought they were moving to Ohio from New York. There were other details that made the situation even more alarming. Law enforcement was involved and indicated that they were not aware of any similar instances locally, but that others may have occurred, but were not reported.
Parents and guardians, I encourage you to share this information with any of your children where it would be developmentally appropriate to do so as we try to protect them from bad actors on social media. Thank you for your assistance in this matter and have a great weekend.
